Lymphedema is an abnormal accumulation of protein-rich fluid that brings about swelling of the body most often in the arms and legs, and occasionally in other parts of the body. The swelling may cause pain and limit how the affected area moves. There is no known cure, but lymphedema can be managed with treatments.
Primary Lymphedema is usually present at birth. Sometimes there are simply too few lymph vessels in the body, or they are too narrow. The lymph transport capacity is then insufficient to remove the accumulating lymph fluid. Secondary Lymphedema is frequently caused by the removal of lymph nodes during breast cancer surgery. The lymph transport capacity is then insufficient to remove the accumulating lymph fluid. Secondary Lymphedema is frequently caused by the removal of lymph nodes during breast cancer surgery. Other causes may include radiation therapy, filariasis, silica dust, infection, and trauma.
Stage 0 (Latent or preclinical stage)
No swelling or other visible evidence of impaired lymph transport is present. The patient could be at risk of developing lymphedema.
Stage 1 (Reversible lymphedema)
Stage 0 (Latent or preclinical stage)
No swelling or other visible evidence of impaired lymph transport is present. The patient could be at risk of developing lymphedema.
Stage 1 (Reversible lymphedema)
There is accumulation of protein-rich edema fluid; The swollen tissues are soft and pitting edema is present that may vanish over night.
Stage II (Spontaneously irreversible lymphedema)
Protein-rich edema fluid increases; Prone to develop frequent infections; Impossible to induce a pit by pressing with the thumb; and increase build up of connective and scar tissue.
Stage III (Lymphostatic elephantiasis)
Extreme increase of swelling; Natural skin folds are extremely deepened; The dermal tissues are progressively hardening; Papilomas of skin appear; and increase build-up of connective and scar tissue.
Lymphedema treatment requires Manual Lymphe Drainage done by a physical therapist, nurse, or massage therapist who have been specially trained in lymphatic anatomy, physiology and pathophysiology. Each MLD sessions decongests the involved quadrant of the body starting with the trunk and then moves on to the swollen extremity.
Meticulous skin care must be done before treatment. The limb must be free of infections or fungal infection.
Compression Therapy will increase tissue pressure and prevent reaccumulation of lymph fluid using special bandages applied to the swollen limb. Bandages are applied after each treatment until the limbs are decongested. This intensive phase could last approximately four weeks when done everyday. After decongestion, a custom-made or off-the-shelf compression garment is recommended which the patient should wear everyday. Remedial exercises are done by the patient wearing the compression bandages or garments to aid the pumping ability of the lymphatic system.
